This form is used to report information necessary to initiate an investigation of alleged discrimination, harassment, sexual or gender-harassment, domestic or dating violence, stalking or retaliation pursuant to the Equal Opportunity, Diversity and Affirmative Action Plan (“EO Plan”) and/or the Student Code of Conduct.
It is unlawful to retaliate against a student, employee or any other person affiliated with the San Bernardino College Community College District for filing a complaint or for cooperating in an investigation of a complaint.
Confidential Resources: Reporting to Student Health Services in person will be confidential- Monday through Friday, 8:00am to 4:30pm (spring and fall hours); Monday through Thursday, 8:00am-4:30pm (summer hours). Counselors are available by appointment by calling (909) 384-4495.
